Documents and Forms

The following documents must be electronically submitted for each camper:

1. Transportation & Baggage (New information for 2024)
2. Permission to Leave Camp
3. Med-o-lark Camper Agreement
4. Activity Preference Sheet
5. Magic Falls Whitewater Rafting Waiver (Required if attending excursion)

The following documents must be downloaded, signed, and submitted to healthcenter@medolark.com:

1. Health Forms (Part 1 must be signed by a Parent/Guardian, Part 2 must be signed by a Physician)
2. Emergency Medication Permission Form
